Leaders for every mastermind group value the thoughts of those who are involved within the group.  You are free to complete whatever part of the survey you wish from one question to all of them.  Your name and contact information is helpful but not required.  Often the leader of a mastermind will benefit from other questions, information or feedback which are are only able to do if we have your contact information.
